Reference Number

A  reference to where the document is stored external to Genota

Source

The source or origin of the document

Document Location

The location where the document is stored
 
If the document is stored on a computer or network then this would be the file path
 
If the document is stored externally then this could be a folder reference or the physical location

Document Type

This is an identifier to the type of document
 
A drop down list provides a list  of common types however any other type can be manually entered

Add button

Clears all elements to enable the creation of a new associated document record

Save button

Save the associated document information to file

File Selection button

This button will display the open file dialog to enable the selection of the document path for populating the DOCUMENT LOCATION element

Comments

Provision to enter comments or notes specific to the associated document
